PBV TRANSITION OF VOUCHER REQUEST FORM

Project-based voucher (PBV) holders may apply for a Section 8 voucher after one year of residence at the PBV property. (Applications are accepted in the form of written requests using this form (MHA’s PBV Transition of Voucher Request Form). Applications are accepted at all times. If the waitlist is closed at the time this form is received by MHA, your name will be added to a waitlist. Applications added while the waitlist is closed will be denied if the PBV tenant moves from the PBV property prior to the new admission to the Section 8 Program.

• MHA limits the amount of transfers of PBV vouchers to Section 8 vouchers to 25% of annual new admissions. The family has violated a family obligation. • The family owes the MHA money. • The family was issued a voucher to relocate within the previous 6 months

A notice to vacate is required to be submitted to the landlord once you approved for a Section 8 voucher

1. Complete and return this form to MHA. 2. MHA will contact you once a voucher is available (the waiting period may be extensive based on the demand for vouchers). 3. A voucher is issued to you. 4. You find a unit for MHA approval
